Social Services Coordinator
Our Social Services Coordinators are passionate, organized, and compassionate towards residents. You would be responsible for organizing, implementing, and evaluating the Social Services program to meet the psycho-social needs of residents. This may include resident assessment, maintenance of resident records, ensuring residents are linked to the specific supportive services they need to continue living independently in the community.
Responsibilities
- Assessing resident needs based on admission social services history, resident’s health, psychological and social needs
- Promoting a positive living experience for our residents
- Ensuring confidentiality by adhering to HIPPA & all other privacy standards
- Promoting a positive work environment by collaborating with community team members
Requirements
- 1+ years of experience in case management required
- 2+ years of social services experience, preferably in a senior care setting
- A Bachelor’s Degree required in social work or related programs such as gerontology, psychology, or recreation therapy
- Social Services certification preferred
Qualifications
- Passionate, organized, and compassionate towards residents
- Experience in managing resident needs and supporting independence
- Strong communication and collaboration skills
- Knowledge of HIPPA and other privacy standards

Pay
Pay Range: $23.00 - 25.00/hour, depending on experience
Schedule
Schedule: PART-TIME Monday-Wednesday at 9:00 AM - 4:00 PM20 hours per week
