Job Summary

We are seeking a positive, compassionate, customer service-focused team member for the Resident Engagement Coordinator-COTA position. This role involves creativity and innovation, focusing on developing and executing a new kind of Wellness and engagement program for our Sub-acute and long-term residents.

Qualifications

  • An associate’s degree required.
  • Certified Occupational Therapy Assistant (COTA) required.
  • Experience working with elderly people.
  • Familiar with regulatory requirements.

Essential Job Functions

Program Development and Execution

  • Create and develop a wellness and engagement program that incorporates occupational therapy and resident needs and interests into the program activities.
  • Update this program monthly.
  • The program should be a comprehensive and flexible engagement plan based on the Occupational Therapy Practice Framework including areas such as ADLs/IADLs; Leisure and play; Social participation; Cognitive stimulation and Sensorimotor activities.
  • Tailoring the programming to different levels of ability.
  • Incorporate therapeutic group activities; functional task-based interventions; life skills and reminiscence programming and Mind-body practices (e.g., yoga, Tai Chi, mindfulness).
  • Coordinate with therapy staff to integrate rehab goals into recreational activities.
  • Monitor resident participation, mood, engagement, and functional progress.
  • This programming should be geared towards group activities.
  • This position is not tied to productivity or treatment minutes.
  • Reevaluate and update programming as needed.

Resident & Family Interaction

  • Welcome and communicate with families about therapy and wellness progress throughout their stay.
  • Build a strong and respectful relationship with residents and encourage residents to participate in engagement activities.
  • Completes resident service and care plans with interdisciplinary team.
  • Completes appropriate MDS sections.
  • Coordinate and assist Case Manager with discharges for certain short-term residents.
  • Attend and participate in care conferences and staff meetings.
  • Follow up with families 72-96 hours after discharge to make sure residents are adapting well back at home.

Collaboration & Training

  • Hire and train staff and volunteers as needed.
  • Plans and organize events monthly and work closely with Food Service and other departments to create joyful and meaningful moments for residents, their family members, and staff.
  • Perform special assignments, projects, and other duties as required.
