Resident Care Coordinator - Senior Living - LPN or RN
About the role
Serve as the primary advocate and care coordinator for Independent Living residents, educating residents and families on care options and supporting their health and wellness needs. Assess resident wellness, coordinate care plans, facilitate referrals, and ensure smooth transitions between hospitals, rehabilitation, hospice, and other healthcare services. Collaborate with physicians, nursing, wellness, therapy, and interdisciplinary teams to provide comprehensive, resident-centered care and appropriate placement within the continuum. Build and maintain relationships with residents, families, healthcare providers, and community partners while connecting residents to internal and external support services. Document resident interactions, health concerns, and care plans while ensuring compliance with HIPAA and all confidentiality requirements. Promote a culture of wellness through health education, emergency response coordination, community presentations, and continuous improvement of resident services. Act as the weekend Manager-on-Duty on a rotating basis.
Requirements
- Valid Tennessee LPN or RN license
- At least 5 years of progressive nursing experience
- Strong communication skills
