About the role
The Resident Care Coordinator (RCC) collaborates with the Director of Nursing to plan, organize, develop, and direct the overall functions of resident care services in accordance with current federal, state, and local standards, guidelines, and regulations.
Responsibilities
- Management Functions
- Assist in developing, maintaining, updating, communicating, and enforcing facility and state-specific written policies and procedures for the nursing service department, including the safety program.
- Maintain a current reference of written nursing material to assist the nursing service department in meeting day-to-day resident needs.
- Communicate nursing services schedules with other departments to ensure continuity of resident care.
- Assist in developing, implementing, and maintaining an ongoing Continuous Quality Improvement and Quality Assurance program for the nursing services department.
- Recommend equipment needs of the nursing department to the Director of Nursing.
- Participate in preparing and planning the nursing service department’s budget and staffing plan.
- Nursing Care Functions
- Conduct daily nursing functions in accordance with current rules, regulations, and guidelines for long-term care facilities.
- Ensure all nursing personnel comply with written policies and procedures, including the safety program.
- Admit, transfer, and discharge residents with proper documentation of care and services provided.
- Participate in the orientation of new residents and family members to the facility.
- Communicate with health care providers in a timely and appropriate manner.
- Utilize nursing personnel qualifications according to the resident’s care plan, wishes, and health care provider orders.
- Complete daily clinical and environmental rounds to evaluate resident care and report significant events.
- Meet and resolve issues with residents and/or family members; report significant issues to the supervisor.
- Participate in facility surveys (inspections) as requested by management.
- Follow clinical programs including risk management, infection prevention, quality, and staff development.
- Communication/Documentation Functions
- Complete initial assessments and develop service plans for assigned residents.
- Update, review, and transcribe service plans to identify goals, problems, approaches, and revisions based on nursing needs and new health care provider orders.
- Communicate with health care providers to review treatment plans, complete orders, and progress notes in accordance with established policies.
- Review resident health records for specific treatments, medication orders, diets, and labs for implementation and follow-up.
- Requisition and arrange for diagnostic and therapeutic services as ordered by the health care provider.
- Communicate department policies and procedures to personnel, residents, and visitors.
- Provide written (24-hour report) and oral reports concerning resident condition changes and other shift activities to nursing management.
- Complete and submit Risk Watch reports for accidents/incidents involving residents or staff.
- Complete and file assessments and other health record documentation as required.
- Report discrepancies in physician’s orders, diet changes, or charting errors to the Director of Nursing.
- Ensure clinically accurate documentation according to policy and procedures.
- Review resident health documentation and progress notes from the interdisciplinary team to ensure the service plan is followed.
- Inform nursing personnel of new admissions and resident care needs; ensure rooms are ready for new admissions.
- Greet newly admitted residents upon arrival.
- Notify the resident’s attending health care practitioner and family of any changes in the resident’s condition.
- Meet with assigned support personnel (Housekeeping, Dining Services, Caregivers, etc.) to plan shift services, programs, and activities.
- Coordinate services with other members of the interdisciplinary team to maintain the resident’s total regimen of care.
- Assist in arranging transportation for residents and other third-party services.
- Medication Administration Functions
- Review the medication administration record for completeness and accuracy; report concerns to your supervisor.
- Dispose of drugs and narcotics as required and in accordance with established procedures.
- Personnel Functions
- Assist in the recruitment and selection of nursing service personnel.
- Develop work assignments, schedule duty hours, and supervise nursing staff to ensure performance meets acceptable nursing standards.
- Assist in staff development and monitor performance formally and informally; use coaching, counseling, and discipline methods as needed.
- Collaborate with the Director of Nursing and Executive Director to resolve staff performance issues, including those leading to termination.
- Complete timely performance evaluations as directed.
- Review complaints and grievances, complete investigations, and work with the Director of Nursing on action plans and resolutions.
- Participate in planning, conducting, and scheduling in-service and orientation training classes to meet state and federal requirements.
Requirements
- Minimum of two (2) years of nursing experience required.
- Minimum of one (1) year of leadership or care coordination experience preferred.
- Current, active nursing license (RN or LPN) in the applicable state, in good standing, required.
- Experience in a long-term care, assisted living, or similar healthcare setting preferred.
- Must meet all applicable federal, state, and local requirements for the position.

Pay
$35-$40 per hour
Working Conditions and Physical Demands
- Potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
- Prolonged periods of standing.
- Ability to lift fifty (50) pounds; moving, lifting, or transferring residents may require lifting up to 75 pounds.
- Ability to exert maximum muscle force to lift, push, pull, or carry patients.
- Ability to use abdominal and lower back muscles over time without fatigue.
- Ability to stand for extended periods, bend, stoop, stretch, twist, sit, and reach.
- Fine motor skills.
