Resident Care Coordinator

Responsibilities

• Carefully maintain resident medical records and ensure the unit operates smoothly.
• Provide and track supplies for the unit.
• Work closely with nursing staff, physicians, and residents' families to provide the best care.
• Report resident concerns to the MD/family/care manager, report incidents to applicable parties, document incidents/changes of condition, request orders.
• Assist with assessments and development of ISPs for residents in coordination with the DOW.
• Maintain all residents' charts and ensure they are complete and up-to-date.
• Audit charts regularly, noting deficiencies and discrepancies.
• Ensure staff have access to resident records including ISPs, face sheets, ER packets, folders for appointments.
• Coordinate appointments and transportation for residents.
• Perform medication cart audits at least monthly.
• Follow company uniform and grooming standards.
• Adhere to all policies and procedures of Eden Senior Care.

Requirements

• Previous experience as a caregiver is required (at least 2 years).
• Must be a CBRF Med passer.
• Familiarity with systems for managing care services is a plus.
• Strong communication and interpersonal skills.
• Excellent organizational and time management abilities.
