We’re hiring Clinical Behavioral Care Managers to join our team using the Collaborative Care Model (CoCM)—a proven, team-based approach that integrates mental health into primary care. It’s designed to make care more accessible, coordinated, and effective, especially for conditions like depression and anxiety.
Responsibilities
- Help support the mental and physical health of a group of assigned patients.
- Work with patients to understand their needs, make care plans, and connect them to helpful services.
- Provide short-term mental health support using proven methods like motivational interviewing or problem-solving strategies.
- Monitor patient progress through an internal EMR system and watch for any changes in their symptoms.
- Work with the Psychiatric Consultant to support medication management, including checking on side effects and how well the treatment is working.
- Join weekly meetings with the care team to review and update patient care plans.
- Identify patients who may need more advanced mental health care and make sure they get the right support.
- Handle other related duties as needed.
Requirements
- Completion of Master's degree in a mental health related field.
- Current unrestricted LCSW in NC, TX, FL, VA, or AR.
- Experience working with virtual patients.
Qualifications
- Strong written and verbal communication skills.
- Exhibit a positive, flexible, and solution-focused attitude.
- Ability to quickly synthesize medical and behavioral health data and formulate effective and evidence-based clinical recommendations.
- Maintain active licensure within state(s) of jurisdiction.
- Strong knowledge of clinical standards of care.
Skills
- Proven ability to use tools like the PHQ-9 to track symptoms and adjust care as needed.
- Strong schedule adherence is key, as we rely on timely follow-ups and coordinated care.
