About the role

This position supports a mix of clinical and operational responsibilities with an Emergency Department focus as well as inpatient responsibilities. The position works 7 shifts in a row followed by 7 shifts off on evening shift. Dublin Methodist was recently designated as a level 3 Trauma Center, and this new position will help support this growth and future expansion efforts.

Pharmacists will collaborate with interdisciplinary teams, attend emergency responses (e.g. code blue, rapid response, STEMI, stroke alert, etc.), evaluate ED cultures on discharged patients, provide oversight of medication reconciliation, precept students, serve as an educational resource, and participate in hospital committees relevant to practice area. Emergency response experience is highly recommended.

Responsibilities

• Committed to high reliability organization principles and demonstrates consistent behaviors to provide the safest and most effective care for all.
• Reviews provider orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts providers to clarify orders as appropriate.
• Dispenses and compounds medications and dispenses drug therapy-related devices in accordance with standards of practice and OhioHealth pharmacy policies.
• Ensures all medications, chemicals, and supplies are dispensed without suboptimal integrity and appropriate labeling for proper administration of medication.
• Interprets prescriptions and determines compatibility in accordance with standards of practice.
• Manages individuals’ drug therapy by initiating, monitoring, and modifying prescriptions issued to patients in collaboration with a provider.
• Provides drug information to providers, other health care professionals, and patients as needed.
• Counsels individuals regarding their drug therapy, including recommending drug therapy-related devices and assisting in the selection of non-prescription drugs for common diseases and injuries.
• Provides associate and learner development and training, including precepting students and residents.
• Directs and guides pharmacy technicians and pharmacy support personnel.
• Leads safety, stewardship, and utilization strategies for designated site(s) in collaboration with the system team.
• Facilitates and manages activities of various site and system committees and clinical guidance councils.
• Develops, enforces, and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient, and appropriate medication therapy use.
• Utilizes data, published best practice standards, and recommendations to identify opportunities for proactive change in the medication-use system to improve patient outcomes.
• Ensures compliance with evidence-based recommendations, national patient safety recommendations, and regulatory standards related to safe and appropriate medication use.

Requirements

• Bachelor of Science or Doctor of Pharmacy degree from an accredited institution (Required).
• Current Ohio pharmacist license (Required).
• ASHP accredited residency or equivalent pharmacy practice experience (especially in ED) is strongly preferred.
• Basic Life Support Certification—current or obtained during the orientation period (unless greater than 50% of scheduled work hours are off-site or not in a patient care setting).
• Advanced-level degree in healthcare field.
• Collaborative practice experience.
• Advanced Cardiac Life Support Certification (may be required to obtain as determined by manager).
• Pharmacokinetic drug dosing experience.
• Drug therapy management.

Schedule

Evening shift: 7 shifts on followed by 7 shifts off.
