The Department of Pharmacy at Mount Auburn Hospital is seeking a Clinical Pharmacist in Emergency Medicine to work decentralized in the Emergency Department. This role supports an expansion in ED Pharmacy coverage, collaborates with a medication reconciliation technician, responds to code/stroke events in the ED, and offers teaching opportunities within our Pharmacy Residency Program.

Schedule

This position operates on a predominantly Monday-Friday schedule, with rotating weekends/holidays, and overnight coverage as needed. Pharmacists are required to staff every fourth weekend.

Responsibilities

• Review and approve medication orders, monitor drug therapies, and provide drug information to healthcare practitioners.
• Improve patient health by providing pharmaceutical care and serving as a resource to physicians, nurses, and other health professionals.
• Assume charge responsibility and supervise/direct support staff.
• Ensure the safety and efficacy of prescribed drug therapy in accordance with state and federal laws and regulations.
• Optimize drug therapy by identifying, resolving, and preventing drug-related problems and improving therapeutic outcomes.
• Participate in drug distribution, centralized pharmacy operations, and patient care rounds as required by assigned shifts.
• Respond to code/stroke events in the Emergency Department.
• Engage in teaching opportunities within the Pharmacy Residency Program.

Requirements

• Ability to work independently with minimal to no direct supervision.
• Effective oral and written communication skills.
• Direct responsibility for medication distribution, IV admixtures, and ensuring the safety and efficacy of prescribed drug therapy.
