Responsibilities
- Provide Case Management and/or CBRS services to adults with mental health, behavioral health, and reintegration needs.
- Complete assessments and assist clients in identifying strengths, barriers, goals, and support needs.
- Develop individualized service plans in collaboration with clients and their support systems.
- Absorb and reinforce independent living skills, coping skills, problem-solving skills, communication skills, and community integration strategies.
- Support clients in reducing barriers that contribute to instability and recidivism.
- Advocate for clients and assist them in navigating complex systems and community resources.
- Collaborate with probation and parole officers, treatment providers, employers, housing agencies, and other community partners.
- Maintain accurate, timely, and compliant documentation in accordance with Medicaid and agency standards.
Qualifications
- Bachelor's degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services, or a related field preferred.
- Relevant experience may be considered in lieu of degree requirements where applicable.
- Experience working with mental health, substance use, justice-involved populations, or community-based services preferred.
- Strong communication, organization, and problem-solving skills.
- Ability to build rapport while maintaining professional boundaries.
- Ability to work independently and collaboratively within a team environment.
- Valid driver's license, reliable transportation, and ability to travel throughout the service area.
- Experience with electronic documentation systems is a plus.
Preferred Experience
- Working with justice-involved or formerly incarcerated individuals.
- Probation, parole, corrections, or community supervision systems.
- Behavioral health and substance use treatment services.
- Housing and employment resource coordination.
- Case management or rehabilitation services.
- Motivational interviewing and strengths-based approaches.
- Community outreach, advocacy, and relationship building.
