Registered Nurse - Weekend Triage RN (Part Time)

About the role

This is a weekend/part-time position at Brighton/Suncrest Hospice, a Community Health Accreditation Partner (CHAP) certified hospice. The role requires a commitment to providing high-quality care to patients and their families.

Responsibilities

• Provide care and services to a group of patients, including visits to private homes, Skilled Nursing Facilities, and assisted living facilities.
• Collaborate with the interdisciplinary team to provide psychosocial assessments, interventions, and knowledge of community resources.
• Work in Triage, managing visits, documentation, and meetings.
• Manage time effectively to prioritize and triage multiple concerns.
• Use an electronic medical records system to document care.

Requirements

• Current Registered Nurse (RN) licensure in the state(s) of practice.
• Works well within a team environment, collaborating with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Can work independently and on your own, managing activities such as phone, email, and written communication, attending meetings with healthcare professionals, and understanding hospice rules and regulations.
• Must be willing to drive with reliable transportation, possess a valid driver's license, and have auto insurance.

Qualifications

• Strong time management skills to handle multiple responsibilities.
• Willingness to learn and utilize an electronic medical records system.

Skills

• Excellent communication and interpersonal skills.
• Knowledge of hospice care and regulations.
