About the role
Hiring Senior Case Managers in Tennessee. PCM is seeking a Senior Case Manager who shares our passion for delivering care.
Responsibilities
- Directly manage and provide care to assigned clients
- Collaborate with regional directors and other personnel to address issues and improve services
- Plan, implement, and evaluate care provided
- Coordinate and manage client care conferences
- Serve as a local clinical resource and support the fulfillment of client home care needs
- Assist and collaborate with staffing coordinators on staffing and scheduling
- Adhere to nursing delegation guidelines
- Ensure compliance with Agency policies
- Document and report on various aspects of care and staff performance
- Participate in marketing activities as requested
- Be available during operating hours and participate in all relevant activities
Qualifications
- Graduate of a state-approved school of professional registered nursing
- Bachelor of Science in Nursing (BSN) preferred
- Two (2) years of nursing experience, including one (1) year in home care or a closely related field
- One (1) year of supervisory and/or case management experience preferred
- Current, unrestricted RN license in the state(s) of practice
- CPR certification required

Pay
$44.00 per hour plus shift differentials, where applicable.
