About the role

Hiring Senior Case Managers in Tennessee. PCM is seeking a Senior Case Manager who shares our passion for delivering care.

Responsibilities

  • Directly manage and provide care to assigned clients
  • Collaborate with regional directors and other personnel to address issues and improve services
  • Plan, implement, and evaluate care provided
  • Coordinate and manage client care conferences
  • Serve as a local clinical resource and support the fulfillment of client home care needs
  • Assist and collaborate with staffing coordinators on staffing and scheduling
  • Adhere to nursing delegation guidelines
  • Ensure compliance with Agency policies
  • Document and report on various aspects of care and staff performance
  • Participate in marketing activities as requested
  • Be available during operating hours and participate in all relevant activities

Qualifications

  • Graduate of a state-approved school of professional registered nursing
  • Bachelor of Science in Nursing (BSN) preferred
  • Two (2) years of nursing experience, including one (1) year in home care or a closely related field
  • One (1) year of supervisory and/or case management experience preferred
  • Current, unrestricted RN license in the state(s) of practice
  • CPR certification required

Pay

$44.00 per hour plus shift differentials, where applicable.
