Location: Avera at Home Hospice – Sioux Falls

About the Role

Responsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.

Responsibilities

• Collects patient health data.
• Analyzes the assessment data in determining nursing diagnoses/problem focus.
• Identifies expected outcomes/goals individualized to the patient.
• Develops a plan of care that prescribes interventions to attain expected outcomes.
• Implements the interventions identified in the plan of care.
• Evaluates the patient’s progress toward attainment of the outcomes.
• Systematically evaluates the quality and effectiveness of nursing practice.
• Collaborates with the patient, significant others, and healthcare providers in providing patient care.
• Utilizes research/evidence-based findings in practice if appropriate.
• Considers factors related to safety, effectiveness, cost in planning, and delivering patient care.

Requirements

• Valid driver’s license and insurability by the Avera Health automobile liability insurance carrier.
• Active Registered Nurse (RN) license in the state of practice.
• Ability to work the specified hours, including visual acuity and effective communication skills.

Preferred Qualifications

• Minimum two (2) years related experience and/or training or equivalent combination of education and experience.

Pay

The pay range for this position is $31.00 - $46.25 per hour, dependent upon experience. May be eligible for a $15,000 sign-on bonus.

Schedule

On-call approximately 2 times per month. Service area includes a 40-mile radius of Sioux Falls and a 30-mile radius of Madison and Flandreau.
