Registered Nurse - Home Hospice

About the role

Responsible to assess, plan, implement, evaluate, supervise and coordinate the home care and hospice care of patients in accordance with professional standards of nursing and policies and procedures of Essentia Health. Provides direct patient care to patients requiring care for medical, diagnostic and emergency conditions.

Responsibilities

• Assesses patients' needs and develops care plans accordingly.
• Plans, implements, evaluates, supervises and coordinates the home care and hospice care of patients.
• Provides direct patient care to patients requiring care for medical, diagnostic and emergency conditions.
• Works 48 hours per pay period plus on-call hours.
• Shifts include Evening: 3:30pm - 12:00am.
• Shift Rotation includes Day, Evening, and Night.
• Significant call obligations.

Requirements

• Current nursing licensure in state(s) of employment.
• Basic Cardiac Life Support (BCLS) certification or ability to become certified within 1 month from date of hire.
• Possession of a valid motor vehicle operator's license as verified through appropriate licensing agency.
• Proof of current motor vehicle insurance.

Qualifications

• Minimum of 1 year clinical nursing experience.
