Registered Nurse (RN) - Visiting Nurse

Role Overview

Provides nursing care to patients in their homes or facilities by utilizing the nursing process in accordance with ANA standards. Responsible for patient assessment, diagnosis, goal setting, care planning, implementation, and evaluation. Collaborates with patients, families, and healthcare teams to deliver personalized, evidence-based care while ensuring safety, effectiveness, and cost-efficiency.

Responsibilities

• Collect and analyze patient health data
• Develop and implement individualized care plans
• Evaluate patient progress and adjust care as needed
• Collaborate with healthcare team and patients
• Utilize research and evidence-based practices
• Ensure safety, quality, and effective nursing practice

Qualifications & Skills

• Valid RN license in the practice state
• Valid driver’s license and insurability
• Effective communication and visual acuity
• Preferred: minimum two years of related experience
