About the Role
Responsible for providing nursing care to patients in their homes or facilities by utilizing the nursing process in accordance with ANA standards. The nurse assesses patient health data, develops and implements individualized care plans, evaluates patient progress, and collaborates with patients and healthcare providers to ensure optimal outcomes.
Responsibilities
- Collect and analyze patient health data
- Identify nursing diagnoses and set individualized goals
- Develop, implement, and evaluate care plans
- Collaborate with patients, families, and healthcare teams
- Utilize evidence-based practices, considering safety and cost-effectiveness
- Ensure quality and effectiveness of nursing care through systematic evaluation
Requirements
- Active RN license in the state of practice
- Valid driver’s license and insurability
- Excellent communication and visual acuity
- Preferred minimum two years of related experience
