- Assess residents' health, educational, and environmental needs upon admission and regularly thereafter.
- Communicate significant changes in resident condition to the interdisciplinary team (IDT) and document assessments accurately.
- Develop, review, and revise personalized care plans collaboratively with physicians, residents, families, and IDT members.
- Implement nursing interventions, including medication administration, treatments, and IV therapies, ensuring safety and timeliness.
- Maintain detailed documentation of all care activities and updates.
- Utilize critical thinking and effective communication, including SBARR, for timely physician notifications.
- Monitor resident progress, identify issues, and engage in quality improvement processes such as QAPI.
- Assist residents with activities of daily living (ADLs), respond to call lights, and support overall resident well-being.
- Participate in ongoing education to stay current with nursing practices.
Qualifications
- Graduation from an accredited nursing program leading to RN licensure.
- Active RN license in the relevant state.
- CPR/AED certification (or willingness to obtain within six months).
- Critical thinking, excellent communication, organizational, and time management skills.
- Physical ability to stand, walk, lift, and move residents and equipment safely.
- Manual dexterity and visual acuity.
