Registered Nurse RN Outpatient Cardiology Clinic

About the Role

This is a full-time position, scheduled Monday through Friday, 8am to 4:30pm. The RN will support our outpatient Cardiology Clinic with patient triage, assessments, wound care, and IV infusions, while actively engaging in care coordination across the continuum. You will provide patient and family education on medications, diagnoses, diagnostic procedures, and healthy living, as well as support subspecialty clinics and complete prior authorizations and appeals as needed. Additional responsibilities include coordinating diagnostic testing, assisting with patient scheduling, conducting pre-procedure and pre-operative calls, and collaborating with all members of the multidisciplinary care team and leadership to deliver exceptional, patient-centered cardiovascular care.

Responsibilities

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
• Review patient history and physical with patient/family and ensure completion within appropriate timeframe.
• Review diagnostics and laboratory data; report abnormal results to physicians and caregivers.
• Complete assessment and reassessments according to patient need and policy.
• Formulate a plan of care, including discharge planning, utilizing assessment data and input from patient, family, and health team.
• Initiate a realistic, measurable plan of care in conjunction with the patient/family.
• Consider physical, cultural, psychosocial, spiritual, age-specific, and educational needs in the plan of care.
• Collaborate with multidisciplinary team members to review and revise the plan of care based on patient needs.
• Implement the plan of care through direct patient care, coordination, delegation, and supervision of the health care team.
• Provide care based on physician orders and the nursing plan of care, in compliance with policies and standards.
• Delegate appropriately and supervise the provision of care by other licensed nurses and personnel.
• Promote continuity of care through accurate and complete communication with other caregivers.
• Evaluate patient and family response to care and teaching; revise the plan of care as needed.
• Evaluate patient progress towards goals and expected outcomes in collaboration with the health care team.
• Document assessment, planning, implementation, and evaluation in the patient record.
• Provide care based on the best evidence available; participate in research activities within clinical practice.
• Participate in unit or facility shared leadership, education, role development, and orientation of personnel, patients, students, families, and visitors.
• Contribute to society through activities that lead to excellent patient outcomes.
• Actively participate in the improvement of national nursing quality indicators and outcomes.
• Interact with all levels of staff, physicians, patients, families, and external contacts to support patient care.
• Synthesize and prioritize data from multiple sources to support patient and family responses to health status changes.

Requirements

• Current, valid RN license in state of practice, temporary/interim RN license, or compact RN licensure.
• BLS certification is required.
• Additional certification or continuing education may be required based on area of practice.
• Experience in the clinical area for which you are applying is desired.
• Must maintain clinical performance competencies appropriate to the area of work.
• Banner Registry and Travel acute care positions require a minimum of one year of experience in an acute care hospital setting within the past 12 months.
• Banner Registry and Travel physician practice positions require a minimum of one year of experience as a Registered Nurse in a physician practice or acute care setting.
• For Home Health settings, must be able to operate a personal vehicle with a valid driver's license and active auto insurance.
• For Behavioral Health clinical settings serving children, must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain it for the duration of employment.
• For Banner Home Care AZ, must possess and maintain an Arizona Fingerprint Clearance Card.
• For BSS Behavioral Health roles in Arizona, must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card.

Preferred Qualifications

• Bachelor's degree preferred.
• Professional certification preferred.
• Additional related education and/or experience preferred.
