What It's Like to Be a CHC Registered Nurse (RN) or Licensed Practical Nurse (LPN)

• Assess and Adapt: Evaluate patient's condition, medical needs, and overall well-being.
• Create and Modify Care Plans: Shape treatment plans based on patient needs and adjust as necessary.
• Teach and Support: Provide guidance and support to patients, whether they need medication advice or wound care instruction.
• Communicate and Coordinate: Keep providers, families, and care teams informed about patient status and ensure timely care.
• Document and Reflect: Record all visits accurately for compliance and future care continuity.

What We're Looking For

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Massachusetts.
• One year of nursing experience (home health or psych preferred).
• A drive to provide compassionate, patient-centered care.
• Reliable transportation for patient visits.

Pay

$35 per visit for an RN or $30 per visit for an LPN with an expectation of two visits per hour on average.
