About the Role
Provides professional nursing care to residents in a patient-focused setting, including assessments, care planning, and evaluations within their scope of practice.
Responsibilities
- Develops and implements nursing care plans based on medical histories and assessments.
- Communicates with healthcare team, reviews physician prescriptions, and coordinates care.
- Provides physical, emotional, psychological, and spiritual support to residents and families.
- Educates residents and families on health conditions, medications, and self-care skills.
- Monitors and re-evaluates resident conditions, modifying care plans as needed.
- Maintains a safe environment by adhering to infection control, medication, and safety protocols.
- Ensures proper operation and stock of resident care equipment and supplies.
- Coordinates admissions, discharges, and transfers.
- Supervises nursing assistants and other staff, maintaining workflow efficiency.
Requirements
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) with an active license in the state of practice.
- Ability to perform essential duties, including effective communication, visual acuity, and hearing.
- Preferred 1-3 years of clinical or long-term care experience.
