Registered Nurse (RN) or Licensed Practical Nurse (LPN) | Long Term Care

Avera is currently seeking Registered Nurses (RN) or Licensed Practical Nurses (LPN) to deliver patient-focused nursing care in a retirement community setting.

Responsibilities

• Performs licensed practical nursing functions and services as directed by the Registered Nurse for all residents.
• Completes resident assessments, implements appropriate interventions, and applies nursing process (assess, plan, implement, evaluate).
• Complies with pharmaceutical policies and procedures, ensuring safe and accurate medication/treatment administration per physician’s orders.
• Administers medications according to facility policy, including competency with Pyxis medication administration system.
• Participates in the care planning process, ensuring resident care aligns with age-specific needs and updates plans as necessary.
• Documents care professionally and communicates effectively to manage resident care delivery.
• Guides staff in meeting resident needs and participates in physician rounds, using SBAR communication for updates.
• Processes and transcribes physician’s orders according to facility policy.
• Seeks educational opportunities, shares knowledge, and assists with new staff orientation.
• Investigates incidents and resident/family complaints, reporting immediately to leadership.
• Facilitates conflict management while respecting diversity and teamwork.

Requirements

• Active Licensed Practical Nurse (LPN) license in the state of practice (required upon hire).
• Ability to work the specified hours and perform essential job functions, including adequate visual acuity and effective communication.
• Preferred: Less than 1 year of experience in long-term care.

Pay

$24.50 - $33.75 per hour, dependent upon experience.

Schedule

• 6:00 PM – 6:30 AM, every third weekend and holidays.
• 24 hours per pay period (every two weeks).
