About the Role
Provides comprehensive nursing care in a long-term care setting by developing and implementing individualized care plans, assessing residents' physical, emotional, and psychological needs, and coordinating with healthcare teams.
Responsibilities
- Review physician prescriptions and communicate care plans.
- Support residents and families with health-related needs and education.
- Develop care plans based on medical histories and resident assessments.
- Educate residents and families on health management and care outcomes.
- Ensure a safe environment by adhering to infection control, medication, and safety protocols.
- Maintain and troubleshoot care equipment and supplies.
- Coordinate admissions, discharges, and transfers.
- Supervise nursing staff workflow.
Requirements
- Active RN license in the practice state.
- Preferred 1-3 years of clinical or long-term care experience.
- Effective communication skills.
- Visual acuity and ability to work as part of a team.
