About the Role

Provides comprehensive nursing care in a long-term care setting by developing and implementing individualized care plans, assessing residents' physical, emotional, and psychological needs, and coordinating with healthcare teams.

Responsibilities

  • Review physician prescriptions and communicate care plans.
  • Support residents and families with health-related needs and education.
  • Develop care plans based on medical histories and resident assessments.
  • Educate residents and families on health management and care outcomes.
  • Ensure a safe environment by adhering to infection control, medication, and safety protocols.
  • Maintain and troubleshoot care equipment and supplies.
  • Coordinate admissions, discharges, and transfers.
  • Supervise nursing staff workflow.

Requirements

  • Active RN license in the practice state.
  • Preferred 1-3 years of clinical or long-term care experience.
  • Effective communication skills.
  • Visual acuity and ability to work as part of a team.
