About the role

The Home Hospital Registered Nurse provides essential inpatient care in the patient’s home using leveraged technology and remote clinical care coordination.

Responsibilities

  • Performs and documents the nursing process for patients receiving care remotely or in-person at their home.
  • Reviews and follows up on patient education, implemented care plans, and coordinated services.
  • Triage and act on changes in patient condition.
  • Ensures quality care during daily in-person visits to patients receiving acute care at home.
  • Identifies and implements medical treatment plans in collaboration with the virtual nursing team and providers.
  • Communicates assessment results, diagnostics, and treatment responses to the team.
  • Coordinates appropriate hospital resources for acute inpatient individuals receiving Home Hospital services.
  • Provides direct bedside patient education and consultation on continuum integration services.
  • Collaborates with interdisciplinary care teams to prevent unnecessary hospital admissions.
  • Ensures diversity, equity, and inclusion strategy implementation through collaboration and initiative.
  • Maintains safety by following key concepts of a high reliability organization.

Requirements

  • Current RN licensure in Virginia or eligible/compact state.
  • AHA BLS HCP Certification or equivalent.
  • Minimum of three (3) years of critical care, acute care, or medical/surgical RN experience, or equivalent clinical experience.
  • Completion of a Bachelor’s Degree in Nursing (or higher) from an accredited program preferred.

Qualifications

  • Diploma or Associates Degree from a professional nursing program recognized by the state’s Board of Nursing.
  • Enrolled in an approved RN to BSN program within two years of start date and completed within five years.

Skills

  • Knowledge of the principles of growth and development of the life span.
  • Ability to assess data reflective of the patient's status and interpret necessary information for equitable care.
  • Experience in critical care, acute care, or medical/surgical settings.
  • Follows documented physician/licensed independent provider orders.
  • Guides practice by the ANA Code of Ethics for Nursing and established national nursing practice standards.
