About the role

The Home Hospital Registered Nurse is responsible for providing essential inpatient care in the patient's home using leveraged technology and remote clinical care coordination. They implement the nursing process through a team nursing model, ensuring quality care and safety standards.

Responsibilities

  • Performs and documents the nursing process, assessing, diagnosing, planning, implementing, and evaluating patient care.
  • Reviews and follows up on patient education, care plans, and coordinated services.
  • Triage and act on changes in patient condition.
  • Ensures quality care during in-person visits, working collaboratively with a virtual nursing team and providers.
  • Provides timely responses during urgent in-person assessments.
  • Diagnoses and treats within the scope of nursing, communicating with the team about assessments, results, and responses.
  • Coordinates appropriate hospital resources for patients during their stay.
  • Provides high-level bedside patient education and consultation.
  • Collaborates with interdisciplinary teams to prevent unnecessary hospital admissions.
  • Ensures diversity, equity, and inclusion strategies are integrated into care.
  • Models safety practices and demonstrates accountability for safety.

Requirements

  • Current RN licensure in Virginia or eligible/compact state.
  • AHA BLS HCP Certification or equivalent.
  • Minimum of three (3) years of critical care, acute care, or medical/surgical RN experience.
  • Completion of a Bachelor’s Degree in Nursing (or higher) preferred.

Qualifications

  • Diploma or Associates Degree from a professional nursing program recognized by the state’s Board of Nursing.
  • Enrolled in an approved RN to BSN program within two years of employment and completed within five years.
  • Minimum of three (3) years of critical care, acute care, or medical/surgical RN experience.

Skills

  • Knowledge of the principles of growth and development of the life span.
  • Ability to assess patient data and interpret it to identify needs and provide equitable care.
  • Experience with leveraging technology and remote clinical care coordination.
  • Strong communication and critical thinking skills.
  • Ability to perform phone triage and communicate effectively with the virtual nursing team.
  • Knowledge of biometric data capture, patient symptom reporting, and telehealth.
  • Experience with coordinating appropriate hospital resources and community resources.
