Registered Nurse (RN) - Home Health - Part-time

About the role

Part-time Registered Nurse (RN) position in the Acton area working an afternoon/evening weekday shift. Provides great earning potential with a competitive base salary and an uncapped incentive plan. Focus on psychiatric and home health patients, helping them remain independent, avoid unnecessary hospitalizations, and receive compassionate care at home.

Responsibilities

• Assess and adapt: evaluate patient's current condition, medical needs, and overall well-being at each visit.
• Create and modify care plans: actively shape patients' treatment plans based on needs, adjusting as conditions change.
• Teach and support: provide medication guidance for psychiatric patients, wound care education for post‑op patients, etc., to help them stay independent at home.
• Communicate and coordinate: keep providers, families, and care teams informed of changes to ensure timely, appropriate care.
• Document and reflect: submit visit notes from home for accurate recording and continuity of care.

Requirements

• Active Registered Nurse (RN) license in Massachusetts.
• One year of nursing experience (home health or psychiatric preferred).
• Drive to provide compassionate, patient‑centered care.
• Reliable transportation for patient visits.
