What It's Like to Be a CHC Registered Nurse (RN)

• Assess and Adapt: Evaluate patient's condition, medical needs, and overall well-being.
• Create and Modify Care Plans: Shape treatment plans based on patient needs and adjust as necessary.
• Teach and Support: Guide patients on medication, wound care, and other aspects of their recovery.
• Communicate and Coordinate: Keep providers, families, and care teams informed about patient status.
• Document and Reflect: Record all visits accurately for compliance and future care.

What We're Looking For

• Active Registered Nurse (RN) license in Massachusetts
• One year of nursing experience (home health or psych preferred)
• A drive to provide compassionate, patient-centered care
• Reliable transportation for patient visits
