Schedule
- Part-time | 16–24 hours per pay period
- Working Friday–Sunday every other week

Pay Range: $35.00–$42.00/hour, based on experience, qualifications, and location.

Responsibilities
- Provide Direct Client Care
- Initiate preventative and rehabilitative nursing procedures to ensure client safety and well-being
- Monitor and report changes in client conditions, including reactions to treatments or medication, to the physician
- Educate, supervise, and counsel clients and caregivers on care needs and support them in managing health challenges at home
- Maintain Accurate Client Records and Billing
- Document all services accurately and promptly using Vivie’s EMR system
- Demonstrate an understanding of all payer sources and authorization requirements
- Complete and document supervisory visits with LPNs and HHAs in compliance with regulatory guidelines
- Ensure Regulatory Compliance and Quality Care
- Organize, develop, and direct skilled nursing services following nursing standards, state and federal regulations, and Vivie’s policies
- Maintain the highest degree of quality care, customer service, and fiscal responsibility within the Home Health division
- Communicate Effectively and Collaboratively
- Maintain clear and compassionate communication with clients, families, physicians, referral sources, and the interdisciplinary team
- Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery
- Other Duties As Assigned
- Participation in team meetings, special projects, or assisting in other areas of the organization to ensure client needs are met efficiently
Requirements
- Current Registered Nurse (RN) license in the state of Minnesota
- CPR required within 6 months of hire
- Valid driver’s license and access to an insured personal vehicle
- Ability to pass state-mandated background check
- Ability to read, write, and speak English proficiently to communicate with clients, families, and team members
- Physical capability to perform job duties, including driving to client locations and providing hands-on care
Travel Requirements
Travel to client homes within designated regions. This role includes supervisory visits with LPNs and HHAs but does not include formal supervisory responsibilities.
