About the role

A Registered Nurse in Home Health at Vivie delivers compassionate and personalized skilled nursing care to clients in their homes. You'll play a key role in enhancing the independence, safety, and well-being of our clients.

Responsibilities

  • Initiate preventative and rehabilitative nursing procedures to ensure client safety and well-being.
  • Monitor and report changes in client conditions, including reactions to treatments or medication, to the physician.
  • Educate, supervise, and counsel clients and caregivers on care needs and support them in managing health challenges at home.
  • Document all services accurately and promptly using Vivie’s EMR system.
  • Demonstrate an understanding of all payer sources and authorization requirements.
  • Complete and document supervisory visits with LPNs and HHAs in compliance with regulatory guidelines.
  • Maintain the highest degree of quality care, customer service, and fiscal responsibility within the Home Health division.
  • Communicate effectively and collaboratively with clients, families, physicians, referral sources, and the interdisciplinary team.

Requirements

  • Current Registered Nurse (RN) license in the state of Minnesota.
  • CPR required within 6 months of hire.
  • Valid driver's license and access to an insured personal vehicle.
  • Ability to pass state mandated background check.
  • Ability to read, write, and speak English proficiently to communicate with clients, families, and team members.
  • Physical capability to perform job duties, including driving to client locations and providing hands-on care.

Qualifications

  • Experience in home health care or related field preferred.
  • Strong clinical skills and knowledge of nursing standards, state and federal regulations, and Vivie’s policies.
  • Excellent communication and interpersonal skills.
  • Ability to manage multiple tasks and prioritize responsibilities.

Skills

  • Effective communication and collaboration skills.
  • Proficiency in using electronic medical records (EMR) systems.
  • Knowledge of state and federal regulations related to home health care.
  • Ability to adapt to different client situations and environments.

Pay

$35.00- $44.00/hour (non-exempt), based on experience, qualifications, and location.

Schedule

Full-time | 80 hours/pay period | Monday - Friday 8:00 a.m. to 4:30 p.m.
