Responsibilities
- Collaborate with care teams to assess patient status, develop and implement care plans, deliver treatments, and provide education.
- Monitor and document patient conditions, advise on additional services, and foster strong patient relationships.
Qualifications
- Active Maine RN license or ability to obtain;
- Minimum of 1 year of acute care or equivalent nursing experience;
- Valid driver’s license;
- Strong technology skills, with EMR experience preferred.
