Registered Nurse (RN) - Home Health

Responsibilities

• Provide patient-centered care in the home setting, including assessment, treatment, education, and management of health conditions.
• Collaborate with multidisciplinary teams to evaluate patient status, develop and implement care plans, and coordinate referrals to community resources.
• Promote continuity of care, prevent illness, and support patient health outcomes.
• Manage a manageable caseload with support from triage and education teams, ensuring quality care with limited on-call and after-hours duties.

Qualifications & Skills

• Active Maine RN license or ability to obtain, at least 1 year of acute or med/surg nursing experience, preferred ED, ICU, CCU, or home care background.
• Valid driver’s license with a clean driving record, strong technology skills, and experience with electronic medical records (EMR) preferred.
