What It's Like to Be a CHC Registered Nurse (RN)
- Assess and Adapt: Evaluate patient's condition, medical needs, and overall well-being.
- Create and Modify Care Plans: Shape treatment plans based on patient needs and adjust as necessary.
- Teach and Support: Guide patients through medication, wound care, and other aspects of their recovery.
- Communicate and Coordinate: Keep providers, families, and care teams informed about patient status.
- Document and Reflect: Record visits accurately for compliance and future care continuity.
What We're Looking For
- Active Registered Nurse (RN) license in Massachusetts
- One year of nursing experience (home health or psych preferred)
- A drive to provide compassionate, patient-centered care
- Reliable transportation for patient visits
