Registered Nurse (RN) Home Care Parttime

OCES is expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, and others. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services, promoting healthy, safe living to help consumers stay active and engaged in their communities.

About the role

We are seeking a dedicated, caring Registered Nurse for our Home Care Program to assess and/or authorize services delivered by provider agencies, aiming to promote coordination and cooperation in forming cost-effective, quality service plans.

Responsibilities

• Determine consumers’ eligibility for programs and services according to procedures and guidelines.
• Establish, review, and revise care plans in accordance with funding source regulations, OCES procedures, and standards.
• In conjunction with care management team members, determine appropriateness for specific service options; promote cost-effective service substitutions; establish the frequency, scope, and duration of services; and jointly authorize appropriate service packages.
• Maintain records and prepare reports as required.
• Stay current with OCES policies, standards, and funding source regulations.
• Serve as Lead Assessment Coordinator; participate in interdisciplinary teams.
• Provide consultation to providers and other community agencies.
• Conduct and/or participate in training and workshops as needed.
• Provide consultation to Home Care Program staff.
• Perform other duties as assigned.

Requirements

• Excellent verbal and written communication skills.

• Strong analytical and problem-solving skills.
• Proficient with Microsoft Office Suite or related software.
• Considerable knowledge of developing assessment care plans.
• Extensive knowledge of the medical, emotional, and social needs of elders and people living with disabilities.
• Associate degree in Nursing or related field (bachelor’s degree preferred).
• Current Massachusetts RN License.
• Two years’ clinical/community-based experience.
• Ability to lift up to 25 pounds at times.
• Ability to travel with reliable means of transportation.
• Personal computer with secure internet access at home.
• Prolonged periods of sitting at a desk and working on a computer.
