What You'll Do
- Determining consumers' eligibility for programs and services according to procedures and guidelines
- Establishing, reviewing and revising care plans in accordance with funding source regulations, OCES procedures and standards
- In conjunction with care management team members, determine appropriateness for specific service options; promote cost-effective service substitutions; establish the frequency, scope and duration of services; and jointly authorize appropriate service packages
- Maintaining records and preparing reports as required
- Maintaining knowledge of current OCES policies and standards, and funding source regulations
- Performing role of Lead Assessment Coordinator; participate in interdisciplinary
- Providing consultation to providers and other community agencies
- Conducting and/or participating in training and workshops as needed
- Providing consultation to HC Program staff
- Other duties as assigned.
