What You'll Do

  • Determining consumers' eligibility for programs and services according to procedures and guidelines
  • Establishing, reviewing and revising care plans in accordance with funding source regulations, OCES procedures and standards
  • In conjunction with care management team members, determine appropriateness for specific service options; promote cost-effective service substitutions; establish the frequency, scope and duration of services; and jointly authorize appropriate service packages
  • Maintaining records and preparing reports as required
  • Maintaining knowledge of current OCES policies and standards, and funding source regulations
  • Performing role of Lead Assessment Coordinator; participate in interdisciplinary
  • Providing consultation to providers and other community agencies
  • Conduct and/or participate in training and workshops as needed
  • Provide consultation to HC Program staff
  • Other duties as assigned.

What You'll Bring

  • Strong analytical and problem-solving skills
  • Proficient with Microsoft Office Suite or related software
  • Considerable knowledge of developing assessment care plans
  • Extensive knowledge of the medical, emotional and social needs of elders and people living with disabilities
  • Associate degree in Nursing or related field (bachelor's degree preferred)
  • Current Massachusetts RN License
  • Two years' clinical/community-based
