About the role

Registered Nurse (RN) responsible for providing patient-centered care across sub-acute rehab, long-term care, and memory care units in a skilled nursing facility.

Responsibilities

• Assess patient needs and develop nursing diagnoses.
• Implement and evaluate individualized care plans.
• Collaborate with interdisciplinary teams to deliver quality care.
• Maintain compassionate, respectful, and clinically excellent care environments.

Qualifications

• Current NYS RN license.
• Associate's Degree in Nursing or Nursing Diploma from an accredited program.
