About the role

Bluegrass Care and Rehab Center is a 124-bed facility providing skilled nursing and long-term care. We are dedicated to providing a team approach and caring team members who attend to our patient’s daily needs. Our therapy department consists of physical, occupational, and speech therapists. Signature HealthCARE is a family-based healthcare company offering integrated services across multiple states.

Responsibilities

  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Absorb residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
  • Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
  • Maintain monitoring and observing residents' physical and emotional status; notify the attending physician and responsible party of any change in condition.
  • Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
  • Complete accurate and timely clinical documentation per facility policy and procedure.
  • Care for resident admissions, transfers, and discharges according to established procedures.
  • Participate in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing or designee.

Requirements

  • Current, active RN license in the state of practice.
  • CPR/BLS certification preferred (American Heart Association or American Red Cross).
  • Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
  • Experience in a supervisory or charge nurse role, preferred.
  • Strong clinical observation, medication administration, and documentation skills.
  • Knowledge of Federal and State long-term care regulations.
  • Effective verbal and written communication skills.
  • Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
  • Experience in a supervisory or charge nurse role.
  • Familiarity with electronic health records (EHR) systems.
  • Proficiency with Microsoft Office applications.

Qualifications

  • Education: Bachelor’s degree in Nursing preferred.
  • Experience: Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.

Pay

Up to USD $44.44/Hr.
