Registered Nurse (RN) Clinical Manager- Home Health

Responsibilities
- Oversee the delivery of clinical and caregiver services, ensuring the highest quality of patient/client care in accordance with home health laws and regulatory standards.
- Work closely with Director of Health Care Services and Administrator and manage the clinical staff doing OASIS admissions, case management.
- Coordinate with a team of RNs, LPNs, CNAs, aides, and others to ensure patient goals are met.
- Oversee QA process for skilled documentation requirements.
- Follow the Home Health orders from PCPs working closely with their office.
- Ensure medical supplies/equipment are delivered, and staff are trained on care procedures.
- Assess patient/client needs, resolve issues, and ensure a high level of satisfaction.
- Effectively manage financial resources related to the delivery of quality patient/client care.
- Build strong relationships with healthcare providers, ensuring seamless coordination of patient care and improving patient outcomes.
- Assess patient conditions and develop individualized care plans, achieving tailored interventions that address specific needs and goals.
- Conduct thorough assessments of home environments for safety hazards and make recommendations for modifications, reducing risk of injury.
- Assist in coordinating home healthcare services such as physical therapy or medical equipment rentals, improving patient access to necessary resources.
- Perform wound care procedures including cleaning, dressing changes, and infection prevention measures, reducing complications and promoting healing.
- Administer medications, treatments, and therapies as prescribed by physicians, following strict protocols to ensure patient safety and compliance.
- Monitor vital signs, track progress, and document observations accurately to maintain up-to-date patient records, supporting effective care decisions.
- Respond promptly to emergency situations such as falls or sudden changes in patient condition, initiating appropriate interventions to ensure patient safety.
- Coordinate with pharmacies to arrange timely delivery of medications, enhancing patient convenience and adherence to treatment plans.
- Evaluate patient progress regularly and adjust care plans accordingly based on observed outcomes, optimizing care effectiveness.
Requirements
- Graduate of an accredited nursing program and active RN or BSN license in Massachusetts.
- Minimum of 1 year of nursing experience in home healthcare/OASIS admissions, recerts, resumptions, and DC’s.
- At least 1 year of supervisory and caregiver management experience.
- CPR certification.
- Extensive knowledge of state and federal home health laws and regulatory standards.
- Excellent management, organizational, problem-solving, communication, and interpersonal skills.
