Clinical Manager - Registered Nurse (RN - Home Health

Responsibilities

• Lead and support an interdisciplinary clinical team, ensuring the delivery of high‑quality, patient‑centered home health care.
• Oversee patient assessments, individualized plans of care, and coordination of physician‑ordered services.
• Supervise, mentor, and evaluate clinical staff while promoting regulatory compliance and best nursing practices.
• Monitor clinical documentation, patient outcomes, and quality metrics to ensure accuracy, timeliness, and agency standards are met.
• Collaborate with physicians and the care team to coordinate referrals, transitions of care, and discharge planning.
• Educate and support patients and caregivers to promote understanding of care plans, disease management, and health goals.
• Lead Quality Assessment and Performance Improvement (QAPI) initiatives and conduct supervisory home visits to ensure exceptional care delivery.

Requirements

• Current RN license in South Carolina or compact multi‑state license; RN license in good standing.
• Graduate of an accredited nursing program.
• Strong background in clinical practice and home care management.
• Preferred: 3+ years of home care experience.
• Preferred: 2+ years of management experience.
