Registered Nurse (RN) Case Manager - Eddy Senior Care / PACE
About the role
RN Case Manager – Full‑Time – Eddy SeniorCare (Latham) – Eddy SeniorCare PACE is a Program of All‑Inclusive Care for the Elderly, dedicated to helping seniors remain in their homes and communities while receiving comprehensive health and social services. This full‑time position focuses on providing community‑based home visits and coordinating care for PACE participants to ensure optimal health outcomes and quality of life.
Responsibilities
- Conduct in‑home assessments and develop individualized care plans.
- Collaborate with physicians, social workers, and other team members to manage participant needs.
- Provide education and support to participants and caregivers.
- Monitor clinical status and adjust care plans as needed.
- Document all care activities in compliance with regulatory and organizational standards.
Qualifications
- Current NYS RN license in good standing.
- Minimum 1 year of nursing experience (homecare or community health preferred).
- Valid NYS driver’s license, clean driving record, and reliable vehicle.
- Strong communication and organizational skills.
- Ability to work independently and travel within the service area.
Schedule
Weekday hours, 8 am to 4:30 pm (Day Shift).
Pay
Pay range: $40.00 – $52.81 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may vary based on location.
