In collaboration with patients/families, social workers, physicians and the interdisciplinary team, the Case Manager provides leadership and advocacy in the coordination of patient-centered care across the continuum to facilitate optimal transitions and progression in care. The Case Manager manages clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge, actively working to identify and eliminate barriers to the delivery of clinical services with the patient, family and care team.
Responsibilities
- Provide leadership and advocacy in coordination of patient-centered care across the continuum

- Collaborate with patients, families, social workers, physicians, and interdisciplinary team
Qualifications
Required
- Completion of an accredited BSN Nursing Program (RN)
- Minimum 2 years of experience in clinical nursing
- Minimum 1 year of experience in case management
- Valid state license as a registered professional nurse
- CPR - Basic CPR certification
Preferred
- Certification in case management by a national accreditation organization (e.g., ACM-RN, CCM)
- Required to obtain within 1 year of hire

Pay
Salary Range: $47.96/hour – $74.34/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
Schedule
Day Shift. The Hospital reserves the right to modify employee schedules as needed.
