Case Manager Registered Nurse

Make a meaningful impact on member health and wellness. We are seeking an experienced RN Case Manager to provide telephonic case management support for members with complex healthcare needs. In this role, you will assess, plan, coordinate, and manage care while helping members navigate resources, improve health outcomes, and overcome barriers to care.
Responsibilities
- Conduct comprehensive telephonic assessments and develop individualized care plans.
- Identify health risks, barriers, and opportunities to improve member wellness.
- Coordinate care, referrals, and transitions to appropriate clinical programs and resources.
- Apply clinical judgment to support members with complex and co-morbid conditions.
- Collaborate with multidisciplinary teams to achieve optimal member outcomes.
- Maintain compliance with regulatory requirements and company policies.
Schedule
- Monday–Friday schedules available between 7:00 AM – 6:00 PM CST
- Flexible schedule selection within business hours
- Choice of 30- or 60-minute unpaid lunch
- No nights, weekends, or holidays currently required (subject to change based on business needs)
Required Qualifications
- Active, unrestricted RN license in state of residence
- Willingness to obtain a Compact RN license (company-paid)
- Minimum 5 years of clinical RN experience
- Associate Degree in Nursing (ADN)
- Ability to work within Monday–Friday business hours
Preferred Qualifications
- 6+ months of Case Management or Utilization Management experience
- Case Management Certification (CCM or equivalent)
- BSN
Pay
The typical pay range for this role is $60,522.00 – $129,615.00. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
