Case Manager Registered Nurse

About the Role

Join our Neuro Unit as an RN Case Manager and make a meaningful impact beyond the bedside. In this full‑time, day‑shift role you’ll help patients and families navigate complex healthcare needs while leading safe, coordinated transitions of care.

Responsibilities

• Leadership role in complex discharge planning and care coordination
• Collaborate with physicians, nurses, MSWs, and other healthcare professionals on the Neuro care team
• Provide individualized, proactive, and safe discharge planning for medically complex neurological patients
• Work with MSWs on psychosocially complex cases
• Advocate and coordinate to help patients and families transition to the next level of care
• Support a mission‑driven environment focused on quality, safety, and patient‑centered care
• Professional growth through exposure to complex neurological conditions, post‑acute care, and community resources

Requirements

• Education: Associate Degree in Nursing (required); BSN preferred
• Experience: Two or more years of healthcare experience preferred; case management or related experience preferred
• Licensure/Certification: Current RN licensure in the state of employment (required)

Skills

• Weekend and holiday coverage, if applicable (required)
• Knowledge/competence with Microsoft Office products and keyboarding (preferred)

Schedule

Monday‑Friday, 8:00 AM‑5:00 PM (full‑time, day‑shift)
