Registered Nurse (RN) - Care Manager - Full-Time

We are seeking a Registered Nurse (RN) Care Manager for a full-time, ongoing position focused on patient care coordination across inpatient, emergency, and outpatient settings. Responsibilities

• Conducting comprehensive patient assessments
• Coordinating multidisciplinary care and transitional planning
• Educating patients and caregivers, arranging after-care services, and involving payers
• Supporting goals such as reducing length-of-stay, preventing readmissions, and improving patient satisfaction
• Performing utilization reviews and documentation
• Collaborating with healthcare teams to evaluate patient needs and ensure safe discharges
• Participating in performance improvement activities
• Adhering to organizational standards and policies

Qualifications

• BSN or MSN required
• Current RN license in New York State
• 1–3 years of clinical nursing experience, preferably in Emergency, Critical Care, or Medical-Surgical settings
• Case management experience and familiarity with Milliman/InterQual criteria preferred
• Knowledge of healthcare financial, regulatory, and payer issues
• Strong computer skills required
• Certifications such as PRI, SCREEN, and Care Management (e.g., CCM or CMC) are preferred or required within specified timeframes

Pay

Salary range between $124,156 and $158,065

Schedule

37.5 hours per week, dayshift, Monday–Friday, with alternating weekends and holidays
