Registered Nurse - Full-Time

Role Overview

Provide and direct home-based nursing care to patients, ensuring compliance with laws, regulations, and agency policies. Collaborate with healthcare professionals, patients, families, and community resources to develop and revise individualized care plans that meet patient needs.

Key Responsibilities

• Deliver high-quality clinical services within scope of practice and infection control standards.
• Coordinate care from admission to discharge, ensuring continuity of care.
• Conduct clinical nursing assessments to verify patient eligibility and medical necessity.
• Develop, implement, and update personalized care plans with input from community providers and physicians.
• Adapt care plans based on patient needs and obtain physician approval for revisions.
• Function effectively in diverse home environments, respecting patient diversity and individual circumstances.

Qualifications & Skills

• Current, unrestricted RN licensure in the applicable state.
• Valid driver's license, vehicle insurance, and reliable transportation or access to public transit.
• Preferred: CPR certification, at least one year of RN experience, strong communication, organizational, and independent working skills.
