Registered Nurse (RN) - Care Manager - Full-Time

About the role

We are seeking a Registered Nurse (RN) Care Manager to collaborate with healthcare teams in assessing patient needs, coordinating care, and facilitating safe discharges.

Responsibilities

• Conducting comprehensive patient assessments
• Coordinating multidisciplinary care and transitional planning
• Supporting length-of-stay reduction, readmission prevention, and patient satisfaction
• Communicating with healthcare team members and documenting clinical activities
• Participating in performance improvement and utilization review activities
• Ensuring adherence to policies and standards
• Supporting geriatric care across inpatient, emergency, and outpatient settings

Qualifications and Skills

• BSN or MSN required
• Current RN license in the state of practice
• 1–3 years of clinical experience, preferably in Emergency, Critical Care, or Medical-Surgical settings
• Previous case management experience and familiarity with Milliman/InterQual criteria preferred
• Knowledge of healthcare financial, regulatory, and payer issues
• Strong computer skills
• Certifications such as PRI, NYS SCREEN, and Care Management (e.g., CCM or CMC) are preferred or required within specified timeframes

Pay

The position offers a competitive salary range.

Schedule

This role involves day shifts, Monday through Friday, with alternating weekends and holidays.
