Responsibilities

• Facilitate medical, surgical, psychosocial, nutritional, and pharmaceutical evaluations
• Provide education and support
• Serve as a patient advocate
• Ensure effective communication within the multidisciplinary team
• Manage documentation adhering to regulatory standards
• Participate in quality improvement activities
• Support program development

Qualifications & Skills

• Associate's or Bachelor's Nursing degree; Bachelor's preferred
• RN licensure with multi-state or single-state certification
• Minimum of 3 years nursing experience
• Certifications in Basic Life Support (BLS) required
• Proficiency in computer use
• Excellent organizational and communication skills
