Registered Nurse (RN)- After-Hours Hospice, Full-Time, 3:00 PM - 11:30 PM

About the role

The Hospice Registered Nurse provides direct nursing care including assessment, plan of care, education, and evaluation for patients certified as having a terminal illness who have elected the Hospice benefit. Duties also include performing patient duties necessary to maintain the flow of patient care, coordinating the delivery of care provided by other care team members and disciplines, and responding to the needs of patients, families, and significant others after business hours, holidays, and weekends.

Responsibilities

• Assesses patients' physical, emotional, and spiritual needs
• Develops and implements individualized care plans
• Provides education to patients and families about their condition and treatment options
• Evaluates patient progress and makes adjustments to the care plan as needed
• Coordinates care with other healthcare professionals and agencies
• Responds to patient needs outside of regular business hours

Requirements

• Current California RN license
• Basic Life Support Card
• Valid California Driver's license and automobile insurance
• Public Health Nurse and Certified Hospice and Palliative Nurse preferred
• BSN/MSN preferred
• 1-year experience as a professional nurse, Medicare Certified Hospice experience preferred

• Knowledge of Hospice Condition of Participation
