Registered Nurse (RN) - Acute Care - Care Coordination Team Lead - Full Time Days

Role: Care Coordination Team Lead (RN) responsible for mentoring and overseeing a care coordination team within a hospital setting. Key Responsibilities: Lead patient assessments, discharge planning, transitional care, and resource coordination; collaborate with interdisciplinary teams; support staff training, workflow management, and process improvement projects; monitor quality indicators and team performance metrics; ensure accurate documentation in electronic health records. Qualifications & Skills: Minimum of 3 years recent care coordination experience; strong interpersonal, communication, and leadership skills; proficiency in care assessment, discharge planning, and resource management; ability to coach, mentor, and lead change effectively; experience with process improvement and data analysis preferred. Certifications & Licenses: Active RN license (single or multi-state); BLS certification required; additional social work or healthcare certifications may be relevant. Additional Duties: Support departmental initiatives, facilitate staff onboarding and training, assist with scheduling and coverage, and promote a patient-centered, collaborative work environment.
