Registered Nurse (RN) - Acute Care - Care Coordination Team Lead - Full Time Days

Role Overview: The Care Coordination Daytime Team Lead (RN) oversees and mentors a care coordination team, supporting patients during hospitalization and in outpatient case management. Key Responsibilities: Conduct comprehensive patient assessments, including physical, psychosocial, and socioeconomic factors, to determine care needs.Develop and implement discharge and transitional care plans, addressing barriers and coordinating with interdisciplinary teams.Provide patient and family education, advocate for patient needs, and respond to resource and insurance inquiries.Document assessments, care plans, and progress accurately in the medical record, ensuring compliance with standards.Lead, coach, and mentor team members, facilitate staffing, and participate in hiring and performance evaluations.Support process improvements, analyze team performance metrics, and contribute to quality and safety initiatives. Qualifications & Skills: Minimum of 3 years recent care coordination experience in a hospital setting, with exposure to multiple medical populations preferred.Strong interpersonal, verbal, and written communication skills, with proficiency in high-stress crucial conversations.Ability to organize team functions, lead change, and promote patient autonomy. Licenses & Certifications: Valid RN license, Basic Life Support (BLS), and multi-state or single-state RN licensure required.
