Responsibilities
- Deliver compassionate clinical care
- Conduct skilled assessments and provide routine, PRN, admission, discharge, and death visits
- Provide hospice care that honors physical, emotional, cultural, and spiritual needs
- Partner with patients, families, caregivers, and interdisciplinary team members to develop and follow a comprehensive care plan
- Educate families on what to expect, pain management, and comfort measures
- Lead with accountability, prioritizing calls and visits with urgency, and ensuring accurate and timely documentation
- Maintain strong communication with the entire team, including managers, physicians, social workers, aides, chaplains, and referral sources
- Support the whole person, providing clinical insight and emotional support to families before and after the patient's death, and participating in bereavement planning as needed
- Perform other duties as assigned
Requirements
- Registered Nurse (RN) license in the state of Minnesota
- Valid driver’s license and insured personal vehicle
- CPR certification (or willingness to obtain within 6 months of hire)
- Strong clinical assessment, documentation, and time-management skills
- Willingness to travel to client homes and respond to patient needs during on-call hours
- Physical capability to perform all essential job functions
- Ability to read, write, and speak English to ensure effective communication with clients, staff, and families

Pay
$35.34 - $46.00 / hour (non-exempt), based on experience, qualifications, and location. This position also includes a $10,000 sign-on bonus and guaranteed pay for 64 hours per pay period.

Full-time | 64 hours/pay period | 7 Days On 7 Days Off | 4:30 pm – 8:00 am

Must be available to provide services in our Fergus Falls and surrounding areas.
