Role Overview
Key Responsibilities
- Perform comprehensive assessments of physical, social, and psychological health, collaborating with interdisciplinary teams.
- Develop individualized care plans involving patients, families, and physicians, including education and follow-up support.
- Lead and coordinate care delivery, delegate tasks appropriately, and manage resources efficiently.
- Evaluate patient responses, outcomes, and satisfaction, revising care plans as needed.
Qualifications & Skills
- Current RN license in the practice state, with ability to transfer compact licenses.
- BSN required within four years if less than 12 months of experience.
- Basic Life Support (BLS) certification.
- Strong patient care, communication, critical thinking, and teamwork skills.
Physical Requirements
- Visual acuity, manual dexterity, mobility for patient handling, and ability to operate equipment.
- Must be able to stand, walk, and lift as needed.
Location & Schedule
This role is located in Denver, Colorado, with scheduled hours of 36 per week.
